In our office, you can expect quality comprehensive dental care for your entire family in a comfortable and caring atmosphere.
We are conveniently located at the corner of NW 43rd Street and 39th Ave. Our office hours are Monday through Thursday from 8:00 AM-5:00 PM by appointment. We do not overbook and we will try not to keep you waiting. If we should have an emergency which could delay your appointment more then a few minutes, we will call you at home or work and let you know. We understand how valuable your time if and treat it with respect.
In return, our cancellation policy requires at least 48 hours notice during regular business hours. If you are scheduled on a Monday, please contact us by the previous Thursday morning. There is a $50 untimely cancellation/missed appointment fee. This charge is not meant as a punishment but a way of compensating the office for lost production time. After two untimely cancellations, we reserve the right to discharge you from the practice.
To safeguard your health, we are exceptionally careful about the disinfection of our treatment rooms and equipment. We adhere to the highest standards of sterilization and disinfection.
During your initial visit, Dr. Skoby will perform a thorough dental examination including periodontal evaluation, a compete set of x-rays, and an oral cancer exam. When significant treatment is necessary, we will have a no charge consultation to discuss options, personal needs, fees, and insurance.

Our office adheres to the strict confidentiality requirements of the Health Insurance Portability & Accountability Act of 1996, HIPAA.
Fees and Insurance
Fees
Following your initial visit, Dr. Skoby will complete an individualized treatment plan. Your treatment plan will include our fees for each service as well as an estimate as to what your insurance will cover and what your portion will be. We will review the treatment plan with you to ensure that you understand the recommended treatment before we proceed. We will try to answer any questions that you might have.
Payment
Payment is expected a the time of service. For your convenience, we accept cash, check, Visa, Mastercard, American Express, and Discover. We also offer a financing option through Care Credit with 6 month no interest or extended payment plans. We will be happy to file your insurance claims for you. You are responsible for what your insurance doesn’t pay. Patients are billed at the beginning of the month on the unpaid balance. A $25 late fee with be assessed on any accounts that have gone 30 days without a payment. Any accounts over 90 days will be turned over to a collection agency.
Insurance
We are a provider for only a few select PPO insurance plans. We will gladly file your PPO insurance plan. We can also provide you with a completed insurance form if you have a plan that reimburses the patient.
